Click on add-on icon 

Select Form Exporter from list of installed add-on

Click on Let's Start to start Form Exporter

Select the app where you want to export you questions

Select/Create the File to which questions to be exported

You can see the name of selected file below the buttons

Click on new window icon to  view selected file

You can change the file by clicking on Change button

You can select single or all fields you want to export

Click on refresh buttons to reload form fields

Click on Export Fields to export selected fields

That it!

Now you can open the selected File to view your exported fields
